Submitting Purchase Orders

Bungalow Software accepts Purchase Orders from Medical and Educational Institutions (hospitals, schools, etc.), subject to a credit check.

Instructions

1. Refer to our printable order form or catalog for prices and item numbers.
2. PO orders must be shipped Priority or Next Day so the shipment can be tracked.
3. Fax your Institutional Purchase Order to 1+(508) 526-0305, or mail it to:
   • Bungalow Software * 2905 Wakefield Dr. * Blacksburg, VA 24060-8184
4. Call 1+ 540-951-0623 and leave your name, phone number and the PO #.  If the PO doesn't arrive for some reason, we'll contact you.
5. Our Federal Tax ID # is : 93-1285669. You can also print a copy of our W9 form.

We reserve the right to refuse a Purchase Order from any institution with an unsatisfactory credit history.
